Personal Development Plans
We compile an individual personal development plan (PDP) for each of our service users. PDP's set out how our staff can best help service users reach their full potential and achieve independence.
Over the last year, Southdown has developed a pioneering new style of personal development planning which focuses on outcomes. It includes a scoring system which measures how successfully service users have been in reaching their goals.
How is a plan put together?
A Support Worker and service user work together to identify practical issues which need to be addressed, together with the service user's social and emotional aspirations. These become a series of goals which the service user, with Southdown's support, works towards. We can help the service user overcome any barriers which are preventing change and provide the practical support the service user needs.
Personal development planning focuses on seven areas which impact on a service user's quality of life and independence. They are:
- Work, learning and daytime activities
- Tenancy sustainment and independent living skills
- Income and money management
- Health and well-being
- Housing options and resettlement
- Social networks and community
Above all, personal development plans are not set in stone – they are adjusted as service users' circumstances change. Each plan is automatically reviewed after six months and service users can request a review at any time.
