Moving On and Resettlement Support

Before anyone leaves a Southdown scheme, we carry out a Graduation from Service Assessment. As well as highlighting the goals achieved, this enables people to give feedback on our services.

We have also introduced a new procedure, as part of our Move On and Resettlement Policy, which ensures that all people leaving us have an agreed resettlement plan. The plan includes information about who to contact – including Southdown – if a former tenant faces difficulties.

In addition, we contact people three months after they leave us to see how well they are coping with independence. We are also investigating setting up a system to see how former tenants are doing six and 12 months after departure.
