We aim to help Service Users to:
· Look after and keep accommodation.
· Plan for and move into new accommodation.
· Regain, develop and maintain some control over their life.
· Develop a more settled way of life.
· Improve quality of life.
· Regain and develop personal and social skills.
· Increase self-confidence and self-esteem.
· Take an active part in leisure and community activities.
· Access other support agencies.
· Access training, education and employment.
· Manage personal care and medication.
· Maintain the cleanliness and hygiene of a home.
· Eat well, in accordance with any dietary needs.
· Shop.
· Perform laundry tasks.
· Provide ongoing or permanent support, which is ineligible for Supporting People funding.
Referral Process
· Applications are accepted via care professionals in East Sussex and by privately funding individuals.
· The referrer should complete a referral form with the applicant, or phone in.
· After we receive the referral form we will meet with you to carry out an assessment to decide what Southdown can do to meet your needs.
· At the Assessment meeting Southdown will evaluate whether we are able to provide you with a Community Support service.
· If you are accepted, then we will start supporting you as soon as the funding is in place, or place you on our waiting list if we have no vacancy at that time. We will keep you fully informed about this.
